Florida HB 545 acknowledges parents rights to review the instructional materials related to reproductive health and disease and submit a written exemption from teaching reproductive health and disease instruction.

Parents should use the Reproductive Health and Disease Exemption Form (Spanish version) to request exemption from reproductive health and disease instruction. The completed form should be submitted to the front office of the student’s school of enrollment.
