2023-2024 Medical Intake Information
Clinic staff will be at the school intaking medication and medical supplies on Aug 9 from 930 - 11:00AM.
๏ปฟ๏ปฟmedication must be in original prescription bottle
๏ปฟ๏ปฟover the counter medication must be unopened and have appropriate dosing for age
๏ปฟ๏ปฟAll paperwork needed will be provided at time of medication drop off
๏ปฟ๏ปฟMedication must be dropped off by a parent or Guardian
