Athletics Paperwork

Our athletic program is here to assist young girls and boys in the process of learning. We strive to teach each participant the value of team work, a strong work ethic, sportsmanship, respect and personal discipline. It is our goal to support academic achievement, self-respect, and good behavior. Our programs are competitive, striving for each player and team to reach its full potential. Tryouts are held for each sport and announced well in advance. Students are required to have current parent permission and physical forms on file before trying out.

If your child plans to tryout for or play any sport for SRMS, please make sure they have the following forms:

  • Updated Parent Permission Form (which must be notarized)

  • Current Physical

  • Concussion and Heat Related Illness Form

  • Consent and Release Form

  • For students with an "older packet" an addendum sheet

As a result of House Bill 241, the middle school and the high school student athletic packets have been updated. Going forward from this date, 6/18/21, only the updated packets are acceptable. Any student with an “older packet” already filled out for the coming school year will need to fill out an addendum sheet. Their packet is still good, they just need to add the addendum sheet. This addendum sheet will need to be attached to that student's athletic packet and kept on file at the school this school year. Students without the updates are considered ineligible to participate.

Completed and notarized athletic packets must be turned into the coach BEFORE tryouts for each sport.

2022 Summer Concussion Baseline Testing

Report on the assigned date for your school at the location and time listed below.  
Make sure that the Consent Form (page 2) is filled out prior to arriving for baseline testing. 


*If you completed a baseline test last year you do not have to retest this year, however you may retest yearly*
