The Affidavit of Compliance with Policy on Athletic Recruiting /GA4 is only required for student who did not start the 9th grade at Crestview High School.  This form is required to be filed with the Athletic Secretary one time:

Designated Accessible Wheelchair and Companion seating areas in the Crestview High School Gymnasium are located along the floor level first row of bleachers on each side of the gymnasium. School staff on duty at events in the gymnasium are available to assist patrons in accessing the designated areas. The designated accessible areas are also marked with handicapped access symbols for easy identification. We are reaching out to all of our home game opponents to let you know that we no longer accept cash payment at our game gates. Starting this school year, all tickets are purchased through GOFAN.CO or on the GOFAN app. Tickets may be purchased at the gate but only with credit/debit cards. 

Please let your students, staff and parents know in advance to avoid any inconvenience for them at the gate. We still accept cash payments for concessions at this time. I am including the QR code for GOFAN. Please note that game tickets are $5.00 for adults, $3.00 for students 6 and up, 5 and under are free, as are active military with ID.
