The Affidavit of Compliance with Policy on Athletic Recruiting /GA4 is only required for student who did not start the 9th grade at Crestview High School. This form is required to be filed with the Athletic Secretary one time:
Designated Accessible Wheelchair and Companion seating areas in the Crestview High School Gymnasium are located along the floor level first row of bleachers on each side of the gymnasium. School staff on duty at events in the gymnasium are available to assist patrons in accessing the designated areas. The designated accessible areas are also marked with handicapped access symbols for easy identification.
