The Okaloosa County School District has the student medical card online for the 2026-2027 school year. All families must complete the annual student medical information and parent consent card online through the Focus Parent Portal. The directions are below:

Complet the Annual Student Medical Information & Parent Consent form.
Submit the form by the end of the first week of school.

Please note:
STUDENTS CANNOT BE SEEN OR RECEIVE ANY SERVICES IN THE SCHOOL CLINIC UNLESS THE ANNUAL STUDENT MEDICAL INFORMATION & PARENT CONSENT CARD HAS BEEN COMPLETED THROUGH THE PARENT PORTAL. This includes routine clinic visits, first aid, and other health services provided by school clinic staff.
