If your child requires medication at school:

• You must complete the Medication Dispersion Form (MIS 5183), bring the medicine to the clinic, and sign it in each time a refill is required.

  • Your child can not bring his or her meds to the clinic and they can not keep them in their book bags.

• Your child will receive his or her medication at the ordered time from the doctor. 

  • If you want to give it at another time, then I will need a new doctor's order for school.  At home, you can give at any time. Not here at school.  

•  Cough drops may be given to the teacher to hold in her desk for the child.

•  Any lotions, creams, sprays, or anything else that is medicated must be signed into the clinic.

•  If your child is caught with these items in their backpacks, they will be removed and only you can come to get them.

• The label on the bottle for prescription medications must be updated and for school times only.  

  • If the bottle says 1x daily it is a give at home

  • If the bottle says 2x daily and not a time specified for school hours then it is an at-home medicine

• For any PRN medications, the medication must be age appropriate and not expired.

•  If the doctor wants the child to take pain relievers for an adult then we need a doctor's note ordering that

•  If a child is asking for pain relievers more than 5 times, the doctor should see the child address the problem.

  • After 5 times we would then need a doctor's note for the child to continue.
